About Healthy & Home
After a period of ill health individuals may benefit from increased support with their wellbeing and social needs whilst recovering and returning to the community from a hospital environment or intermediate care provision. The service will support patients and residents with social welfare and holistic wellbeing needs ensuring they can remain or return home safely.
The project team will have responsibility for organising and delivering tasks which facilitate a speedy hospital discharge engaging with a wider network of VCFSE expertise and services. Tasks can be both practical and holistic, i.e. moving furniture, shopping for one of essentials, linking in to community activities or support.
The service will be operational 7 days a week with post holders working at weekends on a rota basis.
Halton and St Helens VCA are seeking proactive and compassionate Healthy & Home Team Members to support patients transitioning from hospital to home in the St Helens area.
These roles are central to improving patient wellbeing and independence by connecting health services with the voluntary and community sector.
2 x Healthy & Home Link Worker (St Helens) – 36 hrs per week – Salary £26,413 pa
The Healthy & Home Link Workers will provide a complementary service to the current discharge pathway at St Helens & Knowsley Teaching Hospitals NHS Trust (STHKTH) and through St Helens Contact Cares. They will enhance the current provision within the hospital and create effective and long-lasting links with the Community and Voluntary Sector (VCS) organisations . The post holder will be a Link Worker integrated within the hospital’s Discharge Team, providing a triage service to volunteer support and local VCS connections. The project will bring the expertise and knowledge of the VCS to the discharge process across Halton. Actual working locations to be agreed a these post will require working from various local sites.
Healthy & Home Support Worker (St Helens) – 20 hrs per week – Salary £12,698 pa
The Healthy & Home Support Worker will provide administrative and practical support to the Link Workers. They will be responsible for taking referrals, recording information, tracking progress and managing relationships with VCFSE organisations who are supporting the service.
