About Healthy & Home

Following a period of ill health, individuals may require additional practical, emotional and social support whilst recovering at home. The Healthy & Home Service works alongside health, social care and community partners to provide personalised support that helps residents regain confidence, maintain their independence and access the services they need to live well.

About the Role

The Healthy & Home Link Worker & Volunteer Coordinator will support people across Halton and St Helens who are returning home from hospital or experiencing increased vulnerability. Working closely with residents, families and partner organisations, you will help identify individual needs and coordinate support that enables people to remain safe, well and connected within their local community. 

The role combines direct support to residents with the coordination of Volunteer Community Champions. This includes developing strong relationships with volunteers, ensuring they feel supported in their role, and helping match people with practical community-based support when it is needed most. 

No two days are the same. One day you may be helping someone access local services and community activities that improve their wellbeing, whilst the next you may be working with volunteers and partners to respond to a resident who requires additional support following discharge from hospital. The role requires a proactive and person-centred approach, with a strong commitment to improving outcomes for local people.

About You

We're looking for someone who has experience of supporting individuals in a health, social care, community or voluntary sector setting and who is passionate about helping people achieve positive outcomes. You'll be confident building relationships, working collaboratively with a range of organisations and supporting volunteers to make a difference within their communities.
