Age UK Mid Mersey is an independent charity, that provides life enhancing services and vital support to anyone aged 50+ that lives in Halton, St Helens, Warrington or Knowsley. They provide a ‘Living Well’ service based around four key themes: Health and Wellness, Social Inclusion, Personal Independence and Staying Connected. They Living Well service is holistic, person-centred and offers a bespoke individual approach that includes practical information, advice, activities and support; that directly impacts the health and wellbeing of their clients. Enabling their clients to achieve a quality of life so they can live independently in the way that they choose..

They are looking for a keen individual to be a Living Well Officer supporting our Wellbeing Model. Must be passionate about supporting adults aged 50+, with an empathetic approach and the ability to provide a range of services on behalf of Age UK Mid Mersey.

Permanent role

Part-time hours: 22.5 per week

Salary: £11.92 per hour

Key duties:

  • Conduct ‘guided conversations’ to identify people’s needs and strengths in order to address existing barriers to their health and wellbeing and to jointly develop an action plan with clients.
  • Liaise with relatives, carers and other agencies/ services as agreed with the individual, to ensure an integrated and personalised approach which makes use of existing individual and community resources to support and care.
  • Motivate individuals to set and achieve personal goals that will improve their quality of life and contribute to their health and wellbeing in the future.
  • Accept and act upon referrals from a wide range of sources, including Age UK Mid Mersey other services, external organisations and individuals themselves.
  • Enable and support adults aged 50+ to make positive changes to improve health and wellbeing outcomes and improve people’s life chances.
