The Beyond Programme is providing strategic leadership and oversight for Children and Young People (CYP) Neighbourhood Multidisciplinary-Team (MDT) implementation across Cheshire and Merseyside (C&M).
NHS England published national guidance, in February 2025, regarding CYP Neighbourhood MDTs. A further Neighbourhood Framework has been released in March 2026. This plan aims to reorganise health and care services around local communities.
This form is designed to gather information about current CYP Neighbourhood activity across Places or Organisations. Whilst terminology and models may differ from national guidance, they would like to hear about any current models (health or non-health focused) you are aware of in C&M. If you know of more than one activity please submit a form for each.
The purpose of this exercise is to allow them to map current activity across the system.
