Please ensure you have read the nomination guidelines before completing this form.

Please note that all sections of the nomination form must be fully completed. We are unable to process incomplete nominations.

It may be helpful to get together contact details for the nominee and the organisation they volunteer with before starting your nomination.

If easier you can prepare your nomination statement in advance and copy and paste it into the relevant sections as you may not be able to save your progress.

Consent must be given by the nominee prior to sharing their details with Halton and St Helens VCA.

The judges will make their decision on the information you provide. Please note that if successful, the words could be used at the awards ceremony.

When writing your submission, please provide at least two examples with some explanation about how your nominee and the role/s they undertake meets the criteria for the award you're nominating them for, and consider the following:

Who benefits from your nominees voluntary work?
What has the impact been of your nominees contribution?
Where does this volunteering take place?
