Transforming Community Services & the VCS - workshops on how you can deliver local NHS services

These events will be held in Halton and St Helens and are intended for groups who expressed an interest in delivering local NHS services through the Transforming Community Services agenda.

You are invited to identify the area you work most in from the following 7 care pathways:

Health & Wellbeing, Children & Families, Acute Care Closer to home, Rehab and Long Term Neurological Conditions, Long Term Conditions, End of Life Care and Mental Health.

There will be 7 sessions across the 2 days to enable local groups to understand the changing agenda and how they might be able to work with the preferred provider in each care pathway to deliver services. More details and how to book can be found on the flyer attached.
